About Our Speakers:
Annette Schmidt Ms. Schmidt is part of the Employer & Quality Association Account Management group involved at national levels with various associations and societies (including CMSA). She has been a committed partner with CMSA at the table and a collaborator to the process of evaluation and discussion with other key stakeholders in healthcare. Health literacy, patient safety and issues of
non-adherence are all tied to a patient centric approach for sanofi-aventis. Through coalitions such as NTOCC the issues can be raised across multiple healthcare leaders in support of improved outcomes - because health matters.
Cheri Lattimer, RN, BSN Ms. Lattimer possesses broad experience and depth of expertise in case management, advanced care management interventions, managed care administration, the development and implementation of integrated medical and behavioral care management systems, continuing education models, contracting, total quality management programs,
medical record documentation and the creation of more efficient models of health care coordination within various health care systems. Currently, Ms. Lattimer is Executive Director for CMSA and the President for CMI, Inc. She is a member of CMLC, DMAA, ACHE and the URAC Board of Directors. She participates on advisory panels for the Society of Hospital Medicine supporting the Heart Failure Initiative and Glycemic Control Program and the Project Director for the National Transitions of Care Coalition. Ms. Lattimer is a frequent lecturer on topics such as management of chronic and high risk
conditions, evolving role of case management, medication adherence, integrated care management programs and information systems, outcomes research studies, disease management, and the role of care management.
Sneak Preview:
The National Transitions of Care Coalition (NTOCC) was formed in 2006 by the Case Management Society of America (CMSA) and sanofi-aventis, US LLC, bringing together thought leaders, patient advocates, and health care providers from various care settings dedicated to improving the quality of care coordination and communication when patients are transferred from one level of care to another. Transitions in care include a patient moving from primary care to specialty physicians; within the hospital it would include patients moving from the emergency department to various departments, such as surgery or intensive care; or when
patients are discharged from the hospital and go home, into an assisted living arrangements or into a skilled nursing facility. NTOCC has 29 participating associations and organizations addressing the critical issues surrounding transitions of care. NTOCC views transitions of care as a major challenge to health care delivery and realizes it can only be solved by breaking down the silos and barriers between different health care settings and working collaboratively for the good of the patient. Whether you are a patient, care giver, health care professional, policy maker or media representative, NTOCC can provide you with
information and tools to better understand, and improve, transitions of care challenges.

About Dr. Demoratz:
Dr. Demoratz has over 22 years experience in counseling the catastrophically ill and injured patient population on "living life under altered circumstances" and end of life issues. He is well known for a direct and focused counseling style in accomplishing his client's goals effectively and efficiently. He has published numerous articles on end of life, bereavement, pain management and catastrophic case management. In 1995, he co-authored the groundbreaking text entitled "Dying 101: A Short Course on Living for the Terminally Ill" and in 2001, he was awarded as one of the national case managers of the year by the National Case Management Society of America. He has presented all over the United States on a wide range of topics to both lay and professional audiences.
